A game appeared on my shelf by itself

Because the site noticed you playing it. When Steam or Discord reports you in a game for more than a couple of minutes, it goes on your shelf so you do not have to.

Turning it off

Settings → Notifications, then switch off Shelve what I play. Nothing is added automatically after that; anything already there stays where it is.

It only adds, it never changes

A game it shelves for you arrives as unstarted. If a game is already on your shelf with a status you chose, this leaves it alone.

Play sessions are a separate thing

Steam playtime also produces session suggestions — "you played this for two hours yesterday, add it to your journal?" — which sit there until you accept or dismiss them. Nothing is written to your journal unless you say so.

If you would rather nothing was watched at all

Disconnect the platform in Settings → Connections. Presence and playtime both come from the link; removing it stops both.
